初代《黑道圣徒2》以其华丽的画面和与GTA相媲美的高自由游戏系统获得了不少游戏玩家，就在THQ对外表示PS3版《黑道圣徒2》取消开发的同时，也表示目前该系列的制作小组Volition将全力投入到本系列续作的开发中，并且跨平台同时发售。
本作仍然是描写美国黑帮之间的为了争夺地盘而发生的斗争。游戏相比前作在画面表现上也进行了一定强化。
发生在原来故事的几年之后。一批新来的自命不凡的黑帮粉墨登场把Stilwater搅得天翻地覆。与此同时，一个叫作Ultor财团的多国集团开始成为城市的轴心，源源不断的把百万美元输入大都会的重建之中。作为圣徒的领袖，你将不得不不计代价的保护你的黑帮成员抵抗新的敌人，包括Ultor财团和黑帮对头。
自由度超高是《黑道圣徒2》的特点。 玩家可以自由决定角色的发展以及游戏的剧情走势，体会“黑道圣徒”系列作品最精髓的部分。故事发生在初代故事后的数年，地点依旧是不平静的Stilwater。 玩家要让圣徒重掌 Stilwater 的权力，同时对那些反抗你的人展开报复。
